How do I return my sample?

After your sample has been collected, use the clear circle sticker to close your collection card. Make sure your label is filled out with your full name, date of birth, and the date of collection. Take the collection card and place it into the specimen bag provided. Then place your packaged sample into the rigid envelope at the bottom of your kit and return via USPS. You can drop it off at the nearest post office or USPS dropbox.
